03 Good Health and Well-BeingAccording to the 2016 Wisconsin County Health Rankings data, there are approximately 3 primary care physicians per 10,000 people in Adams County compared to more than 8 primary care physicians per 10,000 people in the state of Wisconsin. This statistic emphasizes the need for improving access to healthcare, a priority shared by the Adams County community. Our project aims to construct a Community Health Worker (CHW) framework for Family Health La Clinica (FHLC) to implement within the Adams County community. This framework will increase client access to healthcare and overcome barriers that prevent individuals from achieving satisfactory health outcomes. More specifically, FHLC clients will have improved self-management and self-care skills from the health education, interventions, and support provided by the CHWs.
